1. Patellofemoral Pain Syndrome (PFPS)
The patellofemoral joint consists of your kneecap (patella) sliding within a groove on your thigh bone (femur). As you bend and straighten your knee, the kneecap should glide smoothly in a subtle C-shaped path.
When the muscles supporting your hip, quad, and foot are out of balance, the kneecap can pull off-center—a concept known as patellar maltracking. This creates excessive pressure on the joint cartilage behind the kneecap, leading to irritation during stairs, squatting, or running.
2. Iliotibial Band Friction Syndrome (ITBFS)
The IT band is a thick strip of fascia that runs down the outside of your thigh, connecting muscles like the gluteus maximus and tensor fascia lata (TFL) to your shin bone.
Unlike muscle tissue, the IT band does not contract on its own. When the attaching hip muscles are weak or overly tight, they pull the IT band taut, causing repetitive friction and irritation along the outside of the knee joint during continuous knee flexion and extension.

Soft Tissue Myth Busted: Should You Foam Roll Your IT Band?
No—foam rolling your IT band directly is largely ineffective and often painful.
Because the IT band is a dense, fibrous connective tissue, you cannot “release” or stretch it with a foam roller. Instead, focus your foam rolling on the contractile muscles that attach to the IT band:
- Tensor Fascia Lata (TFL): Rolling the front-outside pocket of your hip.
- Gluteus Maximus & Medius: Target the side and back of your hip.
