If you’ve ever dealt with persistent joint or muscle pain, you’ve likely heard terms like tendinitis, tendinosis, or tendinopathy. While these sound similar, their underlying causes—and how they must be treated—are drastically different.
Getting an accurate diagnosis is the critical first step in tendon recovery. Treating a degenerative tendon injury like simple inflammation can delay your recovery for months.

What Is a Tendon?
A tendon is a tough, flexible band of dense fibrous tissue that connects muscle to bone. Tendons transfer the mechanical force generated by your muscles to your bones, enabling movement at your joints.
Tendons are primarily made of Type I collagen fibers aligned in parallel strands. Compared to muscles or bone, tendons have a relatively limited blood supply. This reduced blood flow is one reason tendon injuries often take longer to heal than muscle strains.
The Difference Between Tendinitis and Tendinosis
The primary distinction between tendinitis and tendinosis comes down to inflammation vs. degeneration.

1. What Is Tendinitis?
Tendinitis refers to an acute injury where the tendon tissue actively becomes inflamed. This often happens after a sudden, sharp increase in physical activity—such as running a much longer distance than usual or spending an entire weekend doing heavy yard work.
- Symptoms: Acute pain, localized swelling, warmth, and tenderness to the touch.
- Underlying Tissue: Micro-tears with an active immune/inflammatory response present.
2. What Is Tendinosis?
Tendinosis refers to chronic, non-inflammatory degeneration of a tendon’s collagen structure. It is typically the result of long-term overuse without adequate tissue recovery.
- Symptoms: Deep, aching pain that persists for months, stiffness (especially in the morning), and localized thickening of the tendon.
- Underlying Tissue: Microscopic inspection shows an absence of inflammatory cells. Instead, collagen fibers lose their organized, parallel structure, becoming frayed, weak, and disorganized.
What About “Tendinopathy”?
Because it is difficult to determine whether active inflammation or tissue degeneration is occurring without advanced imaging or biopsy, physical therapists often use the umbrella term tendinopathy to describe any painful, symptomatic tendon condition.
Where Can These occur?
Any muscle in the body; however, some tissues more common:
- Achilles Tendinopathy
- Rotator Cuff Tendinopathy
- Lateral & Medial Epicondylitis (Elbow)
- Patellar Tendinopathy / “Jumper’s Knee” (Knee)
- Gluteal Tendinopathy / Greater Trochanteric Pain Syndrome (Hip)
- De Quervain’s Tenosynovitis (Wrist/Thumb)
Why Diagnosis Matters: How Treatment Differs
Treating tendinitis and tendinosis requires opposite approaches during the early phases of rehabilitation.
Treating Acute Tendinitis (Inflammation)
When a tendon is acutely inflamed, the immediate goal is to decrease pain and control excessive inflammation:
- Short-Term Protection: Temporarily scaling back activities that strain the tendon.
- Targeted Modalities: Ice and conservative anti-inflammatory management (short-term) to manage acute discomfort.
- Gentle Range of Motion: Maintaining joint mobility without overloading the damaged tissue.
Treating Chronic Tendinosis (Degeneration)
Because tendinosis lacks active inflammation, treatments like ice or anti-inflammatory medications are largely ineffective long-term. Healing a degenerative tendon requires progressive mechanical loading to stimulate collagen remodeling.
- Eccentric Exercise: Controlled lengthening of the muscle-tendon unit under load (e.g., slowly lowering the heel off a step for Achilles tendinosis). Research shows eccentric loading stimulates new collagen synthesis and restores tendon capacity.
- Targeted Strengthening: Slowly introducing heavy, slow resistance (HSR) training to rebuild tissue tolerance.
- Correcting Biomechanical Drivers: Addressing mobility deficits, muscle imbalances, or movement errors elsewhere in the kinetic chain.
Examples of Targeted Tendon Loading Exercises
Depending on the site of your injury, specific movement strategies are used to rebuild tendon strength safely:
- Achilles Tendinopathy: Slow, controlled single-leg heel drops off the edge of a step.
- Tennis/Golfer’s Elbow (Epicondylitis): Slow eccentric wrist extension or flexion using a lightweight dumbbell or resistance bar.
- Patellar Tendinopathy (Jumper’s Knee): Slow, loaded squats or decline board squats emphasizing a 3-to-4 second lowering phase.
Note: While temporary discomfort during exercise can be a normal part of tendon remodeling, exercises should never cause sharp or worsening pain. Always consult a licensed physical therapist before beginning an exercise program.
